Participant Release and Waiver of Liability Form

This Release and Waiver of Liability releases the Long Creek Community Club (the “Club”), a nonprofit Community Service Club organized and existing under the laws of the United States as a Section 501(c) (3) tax exempt corporation, each of its directors, officers, employees, and agents.

I release and forever discharge and hold harmless the Club and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from the activities as a participant with the Club, including claims arising out of negligence.

I understand and acknowledge that this Release Discharges the Club from any liability or claim that we may have against the Club with respect to bodily injury, personal injury, illness, death, or property damage that may result from the Participant’s involvement in the Club’s activities.

I grant and convey to the Club all right, title, and interest in any and all photographs, images, video or audio recordings of the Participant or his or her likeness or voice made by the Club in connection with the Participant’s involvement in Club’s events, including but not limited to, any royalties, proceeds, or other benefits derived from such photographs or recordings.
